While a kidney transplant helps people feel better and live longer, individuals must attend lifelong, regular check-ups with doctors specializing in kidney transplantation. These doctors make sure the transplanted kidney is working and handle complex medical issues related to the kidney transplant.<\/p>\n<p>In Canada, more people than ever live with kidney transplants. While this is great news, it also means more people with a kidney transplant need life-long follow-up care. However, many transplant clinics are already very busy. With only six transplant centres in Ontario, some patients must travel hundreds of kilometers to receive their care. For patients in Northern Ontario, attending these visits may be particularly expensive and difficult.<\/p>\n<p>The overall goal of our work is to generate evidence to improve the delivery of patient-centred post-transplant kidney care in Ontario.
