Cette technologie vise \u00e0 \u00ab reconditionner \u00bb les organes endommag\u00e9s pendant le processus de mort en faisant recirculer du sang oxyg\u00e9n\u00e9 dans le corps du donneur apr\u00e8s sa mort, mais avant que les organes ne soient r\u00e9cup\u00e9r\u00e9s pour \u00eatre transplant\u00e9s. Malgr\u00e9 sa promesse d&rsquo;am\u00e9liorer la qualit\u00e9 et la quantit\u00e9 d&rsquo;organes disponibles pour la transplantation, la CRN s&rsquo;\u00e9carte des processus habituels de don de corps et son utilisation est controvers\u00e9e. Notre \u00e9tude identifiera les questions \u00e9thiques li\u00e9es \u00e0 la CRN et s&rsquo;engagera aupr\u00e8s des principales parties prenantes pour d\u00e9terminer comment la mise en \u0153uvre de la CRN au Canada pourrait avoir un impact sur la confiance dans les syst\u00e8mes de don et de transplantation de corps d\u00e9c\u00e9d\u00e9s. Nos conclusions contribueront \u00e0 l&rsquo;\u00e9laboration d&rsquo;une politique r\u00e9gissant la CRN au Canada, ouvrant ainsi la voie \u00e0 l&rsquo;adoption responsable d&rsquo;une technologie permettant de sauver des vies dans le domaine du don d&rsquo;organes.<\/p>\n<\/blockquote>\n<h6><strong>Comment le PRDTC a-t-il soutenu le projet ?<\/strong><\/h6>\n<blockquote>\n<p>Notre \u00e9tude n&rsquo;aurait pas \u00e9t\u00e9 possible sans une bourse d&rsquo;innovation du PRDTC accord\u00e9e en juillet 2021. Gr\u00e2ce \u00e0 cette bourse, notre \u00e9quipe a pu lancer un programme de recherche qui, nous l&rsquo;esp\u00e9rons, d\u00e9bouchera sur des avantages directs pour les Canadiens. Et sans le soutien des r\u00e9seaux et du personnel d\u00e9vou\u00e9 du PRDTC, il n&rsquo;aurait pas \u00e9t\u00e9 possible de r\u00e9unir notre \u00e9quipe hautement interdisciplinaire de chercheurs et d&rsquo;un patient partenaire. De plus, le PRDTC a contribu\u00e9 \u00e0 placer la discussion sur les obstacles \u00e0 CRN au Canada en t\u00eate de l&rsquo;agenda de recherche, par exemple en incluant une session sur l&rsquo;\u00e9thique de la CRN lors du Congr\u00e8s scientifique annuel du PRDTC en 2021 et en invitant notre \u00e9quipe \u00e0 pr\u00e9senter notre protocole de recherche lors d&rsquo;une r\u00e9union du Th\u00e8me 1. Le soutien du PRDTC a contribu\u00e9 \u00e0 mettre notre \u00e9quipe en position de mener avec succ\u00e8s une recherche qui aidera \u00e0 pr\u00e9server la confiance dans les syst\u00e8mes de don et de transplantation.<\/p>\n<\/blockquote>\n<h6>Comment le PRDTC pourrait-il soutenir les orientations futures de ce travail ?<\/h6>\n<blockquote>\n<p>Les conclusions de notre \u00e9tude ne se traduiront pas en elles-m\u00eames par une politique r\u00e9gissant la mise en \u0153uvre et l&rsquo;utilisation de la CRN. Pour que notre \u00e9tude ait l&rsquo;impact positif que nous envisageons, un programme d&rsquo;application et de partage des connaissances est essentiel. Le soutien du PRDTC sera d\u00e9terminant \u00e0 cet \u00e9gard. Nous esp\u00e9rons que le PRDTC pourra nous aider dans nos activit\u00e9s de transfert des connaissances, qu&rsquo;il communiquera nos r\u00e9sultats par le biais de ses diff\u00e9rents canaux afin d&rsquo;informer le d\u00e9veloppement des politiques et qu&rsquo;il nous mettra en contact avec les parties prenantes qui devront \u00eatre impliqu\u00e9es si l&rsquo;adoption de la CRN doit progresser. Although not yet used in Canada, normothermic regional perfusion (NRP) is a perfusion technology used postmortem with cDCDD donors to selectively restore perfusion of oxygenated blood to target organs in situ, reversing ischaemic injury and improving organ viability and post-transplant outcomes. However, NRP poses significant ethical challenges. To preserve trust in deceased donation, these ethical challenges must be addressed to the satisfaction of Canadian stakeholders before NRP\u2019s implementation. This study will identify ethical issues pertaining to NRP and explore perspectives of NRP among key stakeholders. By developing an explanatory framework delineating how stakeholder perceptions of NRP\u2019s ethical implications impact trust in Canada\u2019s donation and transplantation systems, this study will inform the development of responsible policy on NRP\u2019s use in Canada.<\/p>\n<p><strong>Methods and analysis<\/strong><\/p>\n<p>This study includes two workstreams. Workstream 1 is a scoping review of medical and bioethical literature to identify ethical issues stemming from NRP. We will apply a common search string across Medline, PubMed (other than Medline) and Embase to identify relevant articles. We will identify grey literature through Google searches, websites of organ donation organisations and consultation with our research network. No date limits will be applied. All peer-reviewed publications, commentaries, editorials or documents that engage with ethical issues in NRP (or conceptual and empirical issues as they relate to these ethical issues) will be included. News articles, conference abstracts and publications not in English will be excluded. Workstream 2 consists of interviews with healthcare providers, institutional stakeholders, organ recipients and deceased donors\u2019 family members (n=24\u201336), as well as focus groups with healthcare providers involved in deceased donation and transplantation (n<em>=<\/em>20\u201332). Charles est un expert internationalement reconnu en mati\u00e8re d&rsquo;\u00e9thique de la recherche. Ses publications sur le devoir de diligence dans la recherche clinique, l&rsquo;analyse \u00e9thique des avantages et des inconv\u00e9nients des \u00e9tudes et la responsabilisation des communaut\u00e9s dans la recherche ont eu une grande influence. De 2008 \u00e0 2013, Charles a codirig\u00e9 une collaboration qui a produit les premi\u00e8res directives \u00e9thiques internationales pour les essais randomis\u00e9s en grappes. De 2014 \u00e0 2018, il a collabor\u00e9 avec le Dr Adrian M. Owen sur un projet explorant l&rsquo;\u00e9thique de la neuroimagerie fonctionnelle apr\u00e8s une l\u00e9sion c\u00e9r\u00e9brale grave. Les travaux actuels de Charles explorent l&rsquo;\u00e9thique de la recherche impliquant des patients en fin de vie imminente, ainsi que les questions \u00e9thiques li\u00e9es au don d&rsquo;organes, notamment l&rsquo;utilisation de la circulation r\u00e9gionale normothermique. Il a \u00e9t\u00e9 titulaire de la Chaire de recherche du Canada en bio\u00e9thique de 2005 \u00e0 2019.
