Notre responsable des communications, St\u00e9phanie Larivi\u00e8re, a pos\u00e9 quelques questions \u00e0 Alshaima \u00e0 ce sujet :<\/p>\n<h6 data-pm-slice=\"1 1 &#091;&#093;\">Comment pensez-vous que la recherche fera progresser le domaine ?<\/h6>\n<blockquote>\n<p>Bien que cette \u00e9tude porte sur une cohorte relativement petite, elle met en \u00e9vidence l&rsquo;incidence \u00e9lev\u00e9e de la st\u00e9atose h\u00e9patique non-alcoolique (NAFLD) chez les transplant\u00e9s du foie, principalement en raison de l&rsquo;ob\u00e9sit\u00e9. Les r\u00e9sultats permettront d&rsquo;am\u00e9liorer la sensibilisation et la surveillance de ce groupe. Ils soulignent \u00e9galement l&rsquo;importance de maintenir une IMC normale dans la p\u00e9riode post-transplantation pour pr\u00e9venir la NAFLD. L&rsquo;\u00e9tude souligne \u00e9galement l&rsquo;importance de l&rsquo;utilisation de tests non-invasifs pour diagnostiquer avec pr\u00e9cision la NAFLD et les maladies h\u00e9patiques associ\u00e9es chez les transplant\u00e9s h\u00e9patiques. Des \u00e9tudes futures seront n\u00e9cessaires pour d\u00e9terminer le r\u00f4le exact de ces tests et pour savoir s&rsquo;ils peuvent potentiellement remplacer la biopsie invasive actuelle du foie comme norme de r\u00e9f\u00e9rence.<\/p>\n<\/blockquote>\n<h6>Comment le PRDTC a-t-il contribu\u00e9 \u00e0 initier ce travail ?<\/h6>\n<blockquote>\n<p>Les soins \u00e0 long terme des transplant\u00e9s h\u00e9patiques ont \u00e9t\u00e9 un sujet de recherche important pour notre \u00e9quipe. Le PRDTC a contribu\u00e9 \u00e0 initier et \u00e0 mettre en \u0153uvre le suivi r\u00e9gulier de nos greff\u00e9s du foie avec des tests de diagnostic non-invasifs pour la NAFLD et la fibrose du foie dans la pratique clinique. En outre, plusieurs recherches ult\u00e9rieures sur les transplant\u00e9s du foie ont d\u00e9coul\u00e9 de cette \u00e9tude.<\/p>\n<\/blockquote>\n<h6>Comment le PRDTC a-t-il soutenu le projet ?<\/h6>\n<blockquote>\n<p>L&rsquo;ensemble de cette \u00e9tude a \u00e9t\u00e9 financ\u00e9 par le PRDTC. Cela a permis \u00e0 deux \u00e9tudiants d&rsquo;\u00eatre premier auteur (moi-m\u00eame) et Patrick Samaha, co-auteur de cette publication. Il est important de noter qu&rsquo;en tant que premi\u00e8re auteure et \u00e9tudiante dipl\u00f4m\u00e9e, j&rsquo;ai pr\u00e9sent\u00e9 l&rsquo;\u00e9tude lors du congr\u00e8s annuel du PRDTC, ce qui a permis de finaliser le projet. De plus, l&rsquo;affiliation de l&rsquo;\u00e9tude au PRDTC a encourag\u00e9 les patients \u00e0 participer.<\/p>\n<\/blockquote>\n<h6>Comment le CDTRP pourrait-il soutenir les orientations futures de ce travail ?<\/h6>\n<blockquote>\n<p>Nous pensons que les tests non-invasifs peuvent jouer un r\u00f4le important dans le contexte post-greffe h\u00e9patique. La pr\u00e9valence de la NAFLD au Canada est en hausse et repr\u00e9sente un lourd fardeau pour le syst\u00e8me de sant\u00e9. Elle est \u00e9galement fr\u00e9quente apr\u00e8s une transplantation h\u00e9patique en raison de la persistance ou de l&rsquo;aggravation des facteurs de risque. Le soutien du PRDTC pourrait aider \u00e0 poursuivre la recherche sur ce sujet et \u00e0 accro\u00eetre la sensibilisation \u00e0 la NAFLD, qui est la maladie du foie la plus fr\u00e9quente chez les Canadiens.<\/p>\n<\/blockquote>\n<\/div><div class=\"fusion-sep-clear\"><\/div><div class=\"fusion-separator fusion-full-width-sep\" style=\"margin-left: auto;margin-right: auto;margin-top:30px;margin-bottom:30px;width:100%;\"><div class=\"fusion-separator-border sep-double sep-solid\" style=\"--awb-height:20px;--awb-amount:20px;border-color:#e0dede;border-top-width:2px;border-bottom-width:2px;\"><\/div><\/div><div class=\"fusion-sep-clear\"><\/div><div class=\"fusion-text fusion-text-2\"><h5 style=\"text-align: left;\"><strong>R\u00e9sum\u00e9 (en anglais)<\/strong><\/h5>\n<p><strong>BACKGROUND <\/strong><\/p>\n<p>Nonalcoholic fatty liver disease (NAFLD) and nonalcoholic steatohepatitis (NASH) seem common after liver transplantation. AIM To investigate incidence and predictors of NAFLD and NASH by employing noninvasive testing in liver transplant recipients, namely controlled attenuation parameter (CAP) and the serum biomarker cytokeratin 18 (CK-18). We also evaluated the diagnostic accuracy of CK-18 and CAP compared to liver histology.<\/p>\n<p><strong>METHODS <\/strong><\/p>\n<p>We prospectively recruited consecutive adult patients who received liver transplant at the McGill University Health Centre between 2015-2018. Serial measurements of CK-18 and CAP were recorded. NAFLD and NASH were diagnosed by CAP \u2265 270 dB\/m, and a combination of CAP \u2265 270 dB\/m with CK- 18 &gt; 130.5 U\/L, respectively. Incidences and predictors of NAFLD and NASH were investigated using survival analysis and Cox proportional hazards.<\/p>\n<p><strong>RESULTS <\/strong><\/p>\n<p>Overall, 40 liver transplant recipients (mean age 57 years; 70% males) were included. During a median follow-up of 16.8 mo (interquartile range 15.6-18.0), 63.0% and 48.5% of patients developed NAFLD and NASH, respectively. On multivariable analysis, after adjusting for sex and alanine aminotransferase, body mass index was an independent predictor of development of NAFLD [adjusted hazard ratio (aHR): 1.21, 95% confidence interval (CI): 1.04-1.41; P = 0.01] and NASH (aHR: 1.26, 95%CI: 1.06-1.49; P &lt; 0.01). Compared to liver histology, CAP had a 76% accuracy to diagnose NAFLD, while the accuracy of CAP plus CK-18 to diagnose NASH was 82%.<\/p>\n<p><strong>CONCLUSION <\/strong><\/p>\n<p>NAFLD and NASH diagnosed non-invasively are frequent in liver transplant recipients within the first 18 mo.
