Received someone else\u2019s prescription? Received the wrong dose of your regular medications? Took your medication incorrectly?<\/p>\n<p>The <a href=\"https:\/\/www.ismp-canada.org\/index.htm\">Institute for Safe Medication Practices<\/a> (ISMP) Canada is an independent not-for-profit organization that works towards preventing harm from medications and would like to hear from patients and families about your experience with medications.<\/p>\n<p>They are <strong>inviting patients and caregivers to be part of a focus group<\/strong> to better understand what patients and caregivers know or would like to know about staying safe with their medication. What would you do if you experienced a medication error?
