Nicholas Murphy, Charles Weijer, and colleagues from organ donation organizations across Canada explores an important ethical question in organ donation: should families (surrogate decision makers) who have consented to organ donation be asked for specific consent before Normothermic Regional Perfusion (NRP) is used on a potential donor in donation after death determination by circulatory criteria.

NRP restores oxygen-rich blood flow to donated organs to repair any damage and help keep them healthy, which may improve transplant success and increase the number of organs suitable for transplantation. While research suggests NRP may offer advantages, it has also raised ethical questions about specific consent from surrogate decision makers. The authors argue that developing a policy governing NRP consent should be a priority for organ procurement organizations.

In this article, the authors examine the arguments for and against requiring specific consent for NRP. Ultimately, the authors find the arguments in favor of specific consent more compelling. Consent for NRP would (1) demonstrate respect for the autonomy of the previously living person; (2) conform with societal norms governing the treatment of dead bodies; (3) display sensitivity to legitimate family concerns; and (4) emphasize transparency to maintain public and health care provider trust in donation systems.

The authors believe that asking families for specific consent before NRP is used is the best approach while this technique is still being introduced into clinical practice. As further research explores NRP, and researchers better integrate the perspectives of patients, families, healthcare providers, and the public, NRP policies can be reviewed and updated to reflect the latest scientific evidence and knowledge of interest-holder values.

Access the publication here: https://www.sciencedirect.com/science/article/pii/S1600613525030618

Access the study infographic here: https://cdtrp.ca/wp-content/uploads/2026/08/CDTRP_NPR_Infographic_FINAL.pdf

Full authorship: Nicholas B.
