However, if comorbidities already exist (for example chronic lung or kidney disease) unfortunately, lifestyle changes won\u2019t do much at the point of COVID-19 infection. Therefore it makes solid organ transplant patients more vulnerable to the COVID-19 disease.<\/p>\n<\/div><\/div><\/div><div class=\"fusion-panel panel-default panel-b554c4ebf31cde294 fusion-toggle-has-divider\"><div class=\"panel-heading\"><h4 class=\"panel-title toggle\" id=\"toggle_b554c4ebf31cde294\"><a aria-expanded=\"false\" aria-controls=\"b554c4ebf31cde294\" role=\"button\" data-toggle=\"collapse\" data-parent=\"#accordion-13063-1\" data-target=\"#b554c4ebf31cde294\" href=\"#b554c4ebf31cde294\"><span class=\"fusion-toggle-icon-wrapper\" aria-hidden=\"true\"><i class=\"fa-fusion-box active-icon awb-icon-minus\" aria-hidden=\"true\"><\/i><i class=\"fa-fusion-box inactive-icon awb-icon-plus\" aria-hidden=\"true\"><\/i><\/span><span class=\"fusion-toggle-heading\">What do we know about pediatrics and the use of the COVID-19 vaccine?<\/span><\/a><\/h4><\/div><div id=\"b554c4ebf31cde294\" class=\"panel-collapse collapse \" aria-labelledby=\"toggle_b554c4ebf31cde294\"><div class=\"panel-body toggle-content fusion-clearfix\">\n<p>The data are trickling in for pediatrics. The PREVenT-COVID study (for more information on this please go to <a href=\"https:\/\/cdtrp.ca\/en\/prevent-covid-national-study-update\/\">this link<\/a>) is going to have a pediatric solid organ transplant group. What we know so far, is that the safety of the COVID-19 vaccine does not appear to be a problem. We were worried about seeing more side effects with possible pericarditis and myocarditis, which we have not seen and the immunogenicity data that we have is actually very good.<\/p>\n<\/div><\/div><\/div><div class=\"fusion-panel panel-default panel-1964ed10fcac47ab8 fusion-toggle-has-divider\"><div class=\"panel-heading\"><h4 class=\"panel-title toggle\" id=\"toggle_1964ed10fcac47ab8\"><a aria-expanded=\"false\" aria-controls=\"1964ed10fcac47ab8\" role=\"button\" data-toggle=\"collapse\" data-parent=\"#accordion-13063-1\" data-target=\"#1964ed10fcac47ab8\" href=\"#1964ed10fcac47ab8\"><span class=\"fusion-toggle-icon-wrapper\" aria-hidden=\"true\"><i class=\"fa-fusion-box active-icon awb-icon-minus\" aria-hidden=\"true\"><\/i><i class=\"fa-fusion-box inactive-icon awb-icon-plus\" aria-hidden=\"true\"><\/i><\/span><span class=\"fusion-toggle-heading\">At an indoor event, if everyone is fully vaccinated, is a mask still required?<\/span><\/a><\/h4><\/div><div id=\"1964ed10fcac47ab8\" class=\"panel-collapse collapse \" aria-labelledby=\"toggle_1964ed10fcac47ab8\"><div class=\"panel-body toggle-content fusion-clearfix\">\n<p>It all depends on what is happening where you live. In general, if everybody is vaccinated, then try to limit the numbers of people. But what we&#8217;re seeing in Alberta with a high level of circulating virus, we are encouraging our solid organ transplant patients to wear masks indoors.<\/p>\n<\/div><\/div><\/div><div class=\"fusion-panel panel-default panel-62380054b76584187 fusion-toggle-has-divider\"><div class=\"panel-heading\"><h4 class=\"panel-title toggle\" id=\"toggle_62380054b76584187\"><a aria-expanded=\"false\" aria-controls=\"62380054b76584187\" role=\"button\" data-toggle=\"collapse\" data-parent=\"#accordion-13063-1\" data-target=\"#62380054b76584187\" href=\"#62380054b76584187\"><span class=\"fusion-toggle-icon-wrapper\" aria-hidden=\"true\"><i class=\"fa-fusion-box active-icon awb-icon-minus\" aria-hidden=\"true\"><\/i><i class=\"fa-fusion-box inactive-icon awb-icon-plus\" aria-hidden=\"true\"><\/i><\/span><span class=\"fusion-toggle-heading\">What are the thoughts on the controlled temporary weaning of one or more immunosuppression drugs to increase the effectiveness of the COVID-19 vaccine in solid organ transplant recipients?<\/span><\/a><\/h4><\/div><div id=\"62380054b76584187\" class=\"panel-collapse collapse \" aria-labelledby=\"toggle_62380054b76584187\"><div class=\"panel-body toggle-content fusion-clearfix\">\n<p>We worry that when reducing our immunosuppression medications, we&#8217;re going to see more rejection of the transplanted organ. I don&#8217;t think we should be encouraging our solid organ transplant recipients to decrease their immunosuppression so they would have more of a response to the COVID-19 vaccine.<\/p>\n<\/div><\/div><\/div><div class=\"fusion-panel panel-default panel-cf343f5f630237bf7 fusion-toggle-has-divider\"><div class=\"panel-heading\"><h4 class=\"panel-title toggle\" id=\"toggle_cf343f5f630237bf7\"><a aria-expanded=\"false\" aria-controls=\"cf343f5f630237bf7\" role=\"button\" data-toggle=\"collapse\" data-parent=\"#accordion-13063-1\" data-target=\"#cf343f5f630237bf7\" href=\"#cf343f5f630237bf7\"><span class=\"fusion-toggle-icon-wrapper\" aria-hidden=\"true\"><i class=\"fa-fusion-box active-icon awb-icon-minus\" aria-hidden=\"true\"><\/i><i class=\"fa-fusion-box inactive-icon awb-icon-plus\" aria-hidden=\"true\"><\/i><\/span><span class=\"fusion-toggle-heading\">For solid organ transplant recipients to be fully vaccinated against COVID-19, is it with two or three doses of the COVID-19 vaccine?<\/span><\/a><\/h4><\/div><div id=\"cf343f5f630237bf7\" class=\"panel-collapse collapse \" aria-labelledby=\"toggle_cf343f5f630237bf7\"><div class=\"panel-body toggle-content fusion-clearfix\">\n<p>The terminology for the vaccine booster shot is an extra dose that applies for non-transplant recipients. To be considered fully vaccinated as a solid organ transplant recipient, it should be three COVID-19 vaccine doses.<\/p>\n<\/div><\/div><\/div><div class=\"fusion-panel panel-default panel-66c294f5b2b6f3d22 fusion-toggle-has-divider\"><div class=\"panel-heading\"><h4 class=\"panel-title toggle\" id=\"toggle_66c294f5b2b6f3d22\"><a aria-expanded=\"false\" aria-controls=\"66c294f5b2b6f3d22\" role=\"button\" data-toggle=\"collapse\" data-parent=\"#accordion-13063-1\" data-target=\"#66c294f5b2b6f3d22\" href=\"#66c294f5b2b6f3d22\"><span class=\"fusion-toggle-icon-wrapper\" aria-hidden=\"true\"><i class=\"fa-fusion-box active-icon awb-icon-minus\" aria-hidden=\"true\"><\/i><i class=\"fa-fusion-box inactive-icon awb-icon-plus\" aria-hidden=\"true\"><\/i><\/span><span class=\"fusion-toggle-heading\">What is your opinion on requiring COVID-19 vaccination for pre-transplant patients to be listed for transplant and if so, with two or three doses of the vaccine? <\/span><\/a><\/h4><\/div><div id=\"66c294f5b2b6f3d22\" class=\"panel-collapse collapse \" aria-labelledby=\"toggle_66c294f5b2b6f3d22\"><div class=\"panel-body toggle-content fusion-clearfix\">\n<p>A lot of centers are looking into considering a requirement for pre-transplant vaccination. I believe the data are coming. We know that the immunogenicity of vaccines, if given pre-transplant, is much better than if it is given post-transplant. We know that the morbidity and mortality of solid organ transplant patients with SARS-CoV2 infection especially in the early period after transplant is high. Some centers have moved to mandating pre-transplant COVID-19 vaccination, but it is not the case everywhere. (Please check with your transplant centre on their COVID-19 vaccine requirements.)<\/p>\n<\/div><\/div><\/div><div class=\"fusion-panel panel-default panel-09261e1809aaf883a fusion-toggle-has-divider\"><div class=\"panel-heading\"><h4 class=\"panel-title toggle\" id=\"toggle_09261e1809aaf883a\"><a aria-expanded=\"false\" aria-controls=\"09261e1809aaf883a\" role=\"button\" data-toggle=\"collapse\" data-parent=\"#accordion-13063-1\" data-target=\"#09261e1809aaf883a\" href=\"#09261e1809aaf883a\"><span class=\"fusion-toggle-icon-wrapper\" aria-hidden=\"true\"><i class=\"fa-fusion-box active-icon awb-icon-minus\" aria-hidden=\"true\"><\/i><i class=\"fa-fusion-box inactive-icon awb-icon-plus\" aria-hidden=\"true\"><\/i><\/span><span class=\"fusion-toggle-heading\">Is there any news related to a COVID-19 vaccine booster shot for those who are solid organ transplant recipients?<\/span><\/a><\/h4><\/div><div id=\"09261e1809aaf883a\" class=\"panel-collapse collapse \" aria-labelledby=\"toggle_09261e1809aaf883a\"><div class=\"panel-body toggle-content fusion-clearfix\">\n<p>There&#8217;s been just one small study that looked at this. The study showed an increase in antibody production. But the problem is that it\u2019s not clear that continuing to give vaccine doses is going to be the answer. What about a fourth dose and a fifth dose of the vaccine? Most of these studies are just measuring antibody response. This is not the only response that we get with a vaccine. There is the T-cell immune response which we don&#8217;t measure when we just check an antibody response. So there are some patients that despite getting a third, fourth, or fifth dose of the COVID-19 vaccine, might still be at risk, lung transplant recipients, for example. In these patients, we need to think about other modalities, either to increase immunogenicity rather than continuing to give boosters, or consider another strategy to help reduce disease.<\/p>\n<\/div><\/div><\/div><div class=\"fusion-panel panel-default panel-e24904865a177b36d fusion-toggle-has-divider\"><div class=\"panel-heading\"><h4 class=\"panel-title toggle\" id=\"toggle_e24904865a177b36d\"><a aria-expanded=\"false\" aria-controls=\"e24904865a177b36d\" role=\"button\" data-toggle=\"collapse\" data-parent=\"#accordion-13063-1\" data-target=\"#e24904865a177b36d\" href=\"#e24904865a177b36d\"><span class=\"fusion-toggle-icon-wrapper\" aria-hidden=\"true\"><i class=\"fa-fusion-box active-icon awb-icon-minus\" aria-hidden=\"true\"><\/i><i class=\"fa-fusion-box inactive-icon awb-icon-plus\" aria-hidden=\"true\"><\/i><\/span><span class=\"fusion-toggle-heading\">I have had my third COVID-19 vaccine dose. How do we know if we have enough antibodies that have developed to help protect me? Is there a test to verify the antibody levels or amount of protection?<\/span><\/a><\/h4><\/div><div id=\"e24904865a177b36d\" class=\"panel-collapse collapse \" aria-labelledby=\"toggle_e24904865a177b36d\"><div class=\"panel-body toggle-content fusion-clearfix\">\n<p>We encourage patients not to get an antibody test. First, not all antibody tests that are available measure antibody response to the COVID-19 vaccine. Some of these antibody response tests just measure if you&#8217;ve been exposed to or infected with SARS-CoV-2. So that is a problem. The other thing is even in those that don&#8217;t have an antibody response, we have the T-cell immune response that we&#8217;re not measuring. The third thing, we don&#8217;t know what these numbers mean and at what level these numbers are protective against a COVID-19 infection. We know that the vaccines will protect solid organ transplant patients compared to being non-vaccinated and that there&#8217;s been a decrease in mortality. But breakthrough infections will still happen. That&#8217;s why I would continue to follow some of these protective measures like social distancing, mask wearing, and hand hygiene.<\/p>\n<\/div><\/div><\/div><div class=\"fusion-panel panel-default panel-f9e1c07aa10b1da58 fusion-toggle-has-divider\"><div class=\"panel-heading\"><h4 class=\"panel-title toggle\" id=\"toggle_f9e1c07aa10b1da58\"><a aria-expanded=\"false\" aria-controls=\"f9e1c07aa10b1da58\" role=\"button\" data-toggle=\"collapse\" data-parent=\"#accordion-13063-1\" data-target=\"#f9e1c07aa10b1da58\" href=\"#f9e1c07aa10b1da58\"><span class=\"fusion-toggle-icon-wrapper\" aria-hidden=\"true\"><i class=\"fa-fusion-box active-icon awb-icon-minus\" aria-hidden=\"true\"><\/i><i class=\"fa-fusion-box inactive-icon awb-icon-plus\" aria-hidden=\"true\"><\/i><\/span><span class=\"fusion-toggle-heading\">What about early COVID-19 infection intervention treatment options for solid organ transplant recipients? Is this an option that is available across Canada?<\/span><\/a><\/h4><\/div><div id=\"f9e1c07aa10b1da58\" class=\"panel-collapse collapse \" aria-labelledby=\"toggle_f9e1c07aa10b1da58\"><div class=\"panel-body toggle-content fusion-clearfix\">\n<p>It&#8217;s an intervention available in Canada. We&#8217;re using more monoclonal antibodies (laboratory-made proteins that mimic the immune system\u2019s ability to fight off harmful pathogens such as viruses) in those that are infected and still early in the infection. The big question is whether in those who don\u2019t respond to the vaccine, whether these monoclonal antibodies could be given to try to prevent infection. Most of these monoclonal treatments are available in Canada through an IV. These are things we need to look more into going forward.<\/p>\n<\/div><\/div><\/div><div class=\"fusion-panel panel-default panel-5b6f6c92475b1548e fusion-toggle-has-divider\"><div class=\"panel-heading\"><h4 class=\"panel-title toggle\" id=\"toggle_5b6f6c92475b1548e\"><a aria-expanded=\"false\" aria-controls=\"5b6f6c92475b1548e\" role=\"button\" data-toggle=\"collapse\" data-parent=\"#accordion-13063-1\" data-target=\"#5b6f6c92475b1548e\" href=\"#5b6f6c92475b1548e\"><span class=\"fusion-toggle-icon-wrapper\" aria-hidden=\"true\"><i class=\"fa-fusion-box active-icon awb-icon-minus\" aria-hidden=\"true\"><\/i><i class=\"fa-fusion-box inactive-icon awb-icon-plus\" aria-hidden=\"true\"><\/i><\/span><span class=\"fusion-toggle-heading\">Does the type of organ that has been transplanted impact the risk of a severe COVID infection? <\/span><\/a><\/h4><\/div><div id=\"5b6f6c92475b1548e\" class=\"panel-collapse collapse \" aria-labelledby=\"toggle_5b6f6c92475b1548e\"><div class=\"panel-body toggle-content fusion-clearfix\">\n<p>Yes, absolutely. We&#8217;ve seen much more severe disease in our lung transplant patients. In the PREVenT-COVID national research study (<a href=\"https:\/\/cdtrp.ca\/en\/prevent-covid-national-study-update\/\">learn more here<\/a>), we&#8217;re recruiting participants with different organ types and different immunosuppression regimens to address some of these questions.<\/p>\n<\/div><\/div><\/div><div class=\"fusion-panel panel-default panel-8d855c8e55f120b93 fusion-toggle-has-divider\"><div class=\"panel-heading\"><h4 class=\"panel-title toggle\" id=\"toggle_8d855c8e55f120b93\"><a aria-expanded=\"false\" aria-controls=\"8d855c8e55f120b93\" role=\"button\" data-toggle=\"collapse\" data-parent=\"#accordion-13063-1\" data-target=\"#8d855c8e55f120b93\" href=\"#8d855c8e55f120b93\"><span class=\"fusion-toggle-icon-wrapper\" aria-hidden=\"true\"><i class=\"fa-fusion-box active-icon awb-icon-minus\" aria-hidden=\"true\"><\/i><i class=\"fa-fusion-box inactive-icon awb-icon-plus\" aria-hidden=\"true\"><\/i><\/span><span class=\"fusion-toggle-heading\">What do we know about variants, the importance of Delta versus the South African variant and others?<\/span><\/a><\/h4><\/div><div id=\"8d855c8e55f120b93\" class=\"panel-collapse collapse \" aria-labelledby=\"toggle_8d855c8e55f120b93\"><div class=\"panel-body toggle-content fusion-clearfix\">\n<p>I don&#8217;t know about specific data in solid organ transplant recipients. However, we know that the Delta COVID-19 variant is much more transmissible in general compared to other variants and can be associated with increased disease. I don\u2019t think that we have enough data in our solid organ transplant recipients to comment on this.\u00a0 But I would expect it to behave the same way.<\/p>\n<\/div><\/div><\/div><div class=\"fusion-panel panel-default panel-bb49e1bfcbb5d62fb fusion-toggle-has-divider\"><div class=\"panel-heading\"><h4 class=\"panel-title toggle\" id=\"toggle_bb49e1bfcbb5d62fb\"><a aria-expanded=\"false\" aria-controls=\"bb49e1bfcbb5d62fb\" role=\"button\" data-toggle=\"collapse\" data-parent=\"#accordion-13063-1\" data-target=\"#bb49e1bfcbb5d62fb\" href=\"#bb49e1bfcbb5d62fb\"><span class=\"fusion-toggle-icon-wrapper\" aria-hidden=\"true\"><i class=\"fa-fusion-box active-icon awb-icon-minus\" aria-hidden=\"true\"><\/i><i class=\"fa-fusion-box inactive-icon awb-icon-plus\" aria-hidden=\"true\"><\/i><\/span><span class=\"fusion-toggle-heading\">Is COVID-19 going to become something we have to live with, like the influenza?<\/span><\/a><\/h4><\/div><div id=\"bb49e1bfcbb5d62fb\" class=\"panel-collapse collapse \" aria-labelledby=\"toggle_bb49e1bfcbb5d62fb\"><div class=\"panel-body toggle-content fusion-clearfix\">\n<p>I would think so. When that&#8217;s going to happen, I don&#8217;t know.
